    • To be completed by BLM personnel at pick-up.

  • Invoice Form [online]
    • Once animals are adopted/sold, please complete this form. You can also use this form for mileage reimbursement and support for mare/jenny and foal pairs, when applicable. Please review Forever Branded Guidebook for specific guidelines.
      • Forever Branded will reimburse mileage to the Branded Adoption Center operator for trips over 500 miles round-trip to approved BLM facilities from the official Branded Adoption Center address once per quarter at the current federal government mileage reimbursement rate. Receipts must be submitted along with the Mileage Reimbursement Form to Forever Branded.
  • Branded Partnerships Transfer Form
    • To be completed by the Adoption Center operator when animals are picked up and transferred to a Branded Partnerships trainer.
  • BLM Foster Care Form [PDF]
    • To be completed (but not signed) with the Branded Partnerships trainer at the time of an animal transferring from an Adoption Center to a Branded Partnerships trainer. Please do not provide any personal identification on the form (i.e., SSN, email, DL number, etc.). Instead, simply state "on file."
  • Adoption/Clinic Event Form
    • Use this form to submit information about official adoption events held at your Adoption Center, clinics, etc., so that we can help you promote them.
  • Classified Listing Form
    • Use this form to submit and share available horses and burros for the classified listing platform and promotion.

    • Once animals are adopted/sold, please complete this form. You can also use this form for mare/jenny and foal support, when applicable. Please review Forever Branded Guidebook for more information.
  • Progress Report Form [online]
    • Please submit one form per month the animal is in training.
  • Training Graduation Email
    • Submit this email when the animal in training can complete all required training maneuvers. You will be required to submit video of the animal performing these maneuvers with someone other than yourself (the trainer).
    • This email should be sent to reporting@foreverbranded.org.
    • Please include the animal freezemark and the trainer's name in the subject line. Please attach video or include a link to a video in the email.
  • Classified Listing Form [online]
    • Please use this form to provide photos, video and information for Forever Branded personnel to populate a classified listing for your animal and aid in promotion.
  • Adopter Information Form [online]
    • This form is to be provided to adopters/purchasers to provide Forever Branded with basic information for tracking purposes and feedback.
  • Adoption/Clinic Event Form
    • Use this form to submit information about your clinics (a requirement of your Branded Partnership status) so that we can help you promote them.
